How to Request a User Name or Password Reset
Providers or Internal users that forget their User ID or password must contact their Organization Administrator. This is the person in your organization who sets up and maintains logon accounts.
Providers
- Forgot User ID/Reset Password.
- Please contact your Organization Administrator, the person listed on your enrollment application if you registered at the time of enrollment to reset your password.
- If the provider is a Provider Organization Administrator, then contact the Provider Relations Unit to reset your password or obtain a forgotten User ID.
Internal Users
- Please contact your Organization Administrator, the person in your organization that sets up and maintains your logon account.
