How to Apply For Medicaid

To apply for Medicaid, you must fill out an Application for Assistance. You can get the Application for Assistance from the Department of Health and Human Services (DHHS) Web site or from a State Office. To access the Web site, do one of the following:

On the Member menu, click How to Apply. The New Hampshire Department of Health and Human Services (DHHS) Web site is displayed. On the How Do I... pod, click Apply for Medicaid? and follow the instructions on the various ways you can apply.
Go directly to the New Hampshire Department of Health and Human Services Web site, and on the right side of the page on the How Do I... pod, click Apply for Medicaid?.

You can also pick up an application in person by going to a DHHS office. You can find a State Office by doing one of the following:

On the Directories menu, click Find a State Office.
Go to the New Hampshire Department of Health and Human Services Web site, on the How Do I... pod click Find my local district office?.

Your application and supporting documentation is reviewed by a DHHS representative to ensure that you qualify for the programs for which you have applied. The DHHS representative will contact you for an interview as part of the eligibility process.

Medicaid eligibility usually begins on the day you have met all the requirements for the program you applied for and if your income falls within the specified resource limits.
