Application Status Response Page

You receive the Application Status Response page when you enter a tracking number to check the enrollment status on your application.

Enrollment Tracking Status Fields

Field	Description
Tracking Number	Tracking number entered on the application request.
Name	Name of individual provider, group provider, or trading partner on the enrollment application.
Date Received	Date the enrollment was received.
Your Application Status is	Current status of the enrollment. Examples: In Process, Pending, Complete
Status Date	Date the status was last updated.
Pending Reason(s)	Reason why the application is still pending.
