How to Register as a Provider

A provider or group practice must have a NH Title XIX provider number and Social Security number (SSN) or Federal Employer Identification number (FEIN) to begin the registration process.

The person who is going to be your Organization Administrator is identified when registering for access to the NH MMIS Health Enterprise Portal. The Organization Administrator is the person who is able to add users, reset passwords, and remove users from your practice.

Note: If you are joining an existing provider or group that has a registered Organization Administrator, please contact that person to set up a user ID and password for you.

To register as a provider

  1. On the public Home page, go to the Provider Registration pod, and then click Register.

    The Web Registration page is displayed.

    Tip: If you accessed the Web Registration page by mistake, click HOME to return to the public Home page.

  2. Enter your NH Title XIX Provider # and your SSN/FEIN that you submitted with your provider enrollment application.
  3. Click Submit.

    Your NH Title XIX provider number is verified and your provider name is displayed. An error message is received if your Organization Administration has already been assigned a user ID. Otherwise, the Organization Administrator panel is displayed so you can continue your registration process.

  4. Enter all organization administrator information, making sure to include all required information (an * asterisk indicates required fields).

  5. Click Register.

    The Confirmation of Registration page is displayed.

    Note: Your assigned user ID and password is mailed to your address on file.

  6. To print your registration confirmation, on the action bar, click Print.
  7. To return to the public Home page of the NH MMIS Health Enterprise Portal, click Return to homepage.
