How to Use Provider EFT Enrollment

The EFT Enrollment link on the Quick Links pod is used to enroll a provider in Electronic Funds Transfer (EFT) so electronic payments can be received.

Optionally, you can:

• Cancel your entries. To close the page or panel without saving your changes, click Cancel.

To enroll in EFT

1. On the Quick Links pod, click EFT Enrollment.
2. On the EFT Enrollment Instructions page, click Continue to enroll in EFT.

3. Complete the fields.

4. Do one of the following:

   • To validate and submit your EFT application, click Validate and Submit.
   • To save your application, click Save.
   • To exit out of the application, click Exit Application.
