Member PIN Request Page

You use the PIN Request page to request a new PIN before logging in to the NH MMIS Health Enterprise Portal because you forgot your PIN number or your account is locked.

PIN Request Fields

Field	Description
Enter Your Member ID	The member identification number provided to you by New Hampshire Department of Health and Human Services when you applied for Medicaid benefits.
Re-enter Member ID	The member identification number provided to you by New Hampshire Department of Health and Human Services when you applied for Medicaid benefits.
