Provider Inquiry Page

You use the Provider Inquiry page, available as a link in the Quick Links pod, to send inquiries, questions or changes in demographic information to the Provider Relations Unit.

Provider Inquiry Fields

Field	Description
To send your inquiry to the Provider Relations Unit, click Submit.
Provider ID	Displays the provider ID associated with the user ID that you used when you logged into the New Hampshire MMIS Health Enterprise Portal.
Provider Name/Institution	Displays the name of the provider or institution associated with the provider ID.
Address	Displays the address on file that is associated with the provider ID.
Inquiry Categories	A category list that can be selected and identifies the type of inquiry you are making and submitting.
Comments	Comments or questions about the inquiry category selected.
