Revalidation Reminder: All providers who are enrolled with New Hampshire Medicaid are required to complete revalidation every 5 years. When you are selected to complete revalidation, you will be notified in advance of the due date by letter via US mail sent to your mailing address currently on your provider record in MMIS. You will also receive an email notification if your NH Medicaid enrollment includes an email contact. You will be required to log into the NH MMIS portal to complete the online revalidation application.
Providers may be subject to claims suspension and then termination of their enrollment if the revalidation is not completed in a timely manner.
